How to update your Summer Program Participant Information form

The Summer Program Participant Information form (Activity Section 16600‑300A) is completed when you enroll a child in a summer program. If you need to make changes after submitting it - such as updating an emergency contact, primary physician, or health information - staff can update the form for you.

Need to update information after you’ve already submitted the form?

You can request updates using one of the following options:

1. Contact the facility offering the program

Call or email the recreation or community center hosting your child’s program.

Staff can quickly update the form on your behalf.
